Information about the medical certificate for the weapon:
In order to obtain a permit to purchase a firearm, one must undergo a preventive health check-up at a personal health care facility and obtain a certificate approved by a family physician. Later, the person needs to undergo periodic health checks in accordance with the law.
Persons wishing to obtain a permit to purchase a weapon must have:
- Identity Card;
- Personal health history (ambulatory card) or form issued by a family doctor no. 027/a for diseases contracted during the last 10 years*.
* For persons who do not have a valid form issued by a family doctor no. 027/a (on the state of health), whether the clinic where the patient is registered is not in the city of Šiauliai, as well as those who are not registered in Lithuania, the mandatory health checkup is performed on site (examinations, electrocardiogram).
